Enrollment
Enrolling as an Individual Member
If your company does not offer Patient Care you can enroll as an Individual Member and receive all of the great benefits and services of Patient Care for one year.
The annual cost is $1,000 for a single individual, $2,000 for a couple and $3,000 for a family.Enrollment requests processed by the 15th day of the month will be effective the 15th of that month.
Enrollment requests processed on the 16th day or later will be effective the 1st of the month.
For example: John Deer fills out our online enrollment form on January 12th. His order is processed on the 12th and his subscription effective date is January 15th. If John had enrolled on the 16th, his subscription effective date would be February 1st.
